# NASH Requirements and Online Application: NASH-OAS Guide

NASH requirements and the NASH-OAS application at nash.deped.gov.ph: documents to upload, each status, the PHP 720 fee and the assessment permit at the SDO.

This is the Markdown copy of [NASH Requirements and Online Application: NASH-OAS Guide](https://nash.com.ph/blog/nash-online-application-requirements), an article on the NASH Reviewer Blog by Mark Anthony Llego, published Oct 26, 2026, updated Nov 4, 2026. Topics: Aspiring school heads. NASH Reviewer is an independent practice platform not affiliated with DepEd, BHROD, or NEAP.

Below are the requirements and the online application for the National Assessment for School Heads (NASH, formerly the NQESH), as the Department of Education (DepEd) ran them for FY 2025 Batch 2: the NASH Online Application System (NASH-OAS) at nash.deped.gov.ph, the documents to upload, every application status, the PHP 720 assessment fee and how to claim the assessment permit at your Schools Division Office (SDO).

DM-OUHROD-2025-3318, "Updates and Instructions on the Administration of the FY 2025 National Assessment for School Heads (NASH) - Batch 2" (November 17, 2025), with its Annexes 1, 2 and 5, set the process. The December 19, 2025 advisory, the NASH-OAS Guide for Applicants and DepEd's official application FAQs add detail. The FY 2025 batches are over, so this page is a record of how Batch 2 was run and a way to prepare now. The memorandum for the next NASH may change any step, fee or document.

## Key points

- **You apply online, with your own DepEd email.** Applicants registered at https://nash.deped.gov.ph and signed in with their official DepEd Google or Microsoft account. Without an active DepEd email you cannot create an account.
- **The details in your account cannot be edited later.** Your name, birth date and other account details go to the DepEd Learning Management System (LMS), the assessment and your results. Corrections need a request through your SDO evaluator to the Regional Human Resource Management Officer (HRMO).
- **Upload clear PDF scans, one file per document, 10 MB at most.** The core set is the IPCRF, service record, certification of relevant experience, transcript or diploma, training certificates and PRC license. School heads in acting roles add the OPCRF and their designation.
- **Two offices check your application.** An SDO evaluator reviews it first, then a Regional Office (RO) validator. Either can return it as "Re-Apply" or mark it "Disapproved".
- **You finish at the SDO, in person.** You bring the originals, two passport photos taken within six months and the PHP 720 fee. Your status changes to "Assessment Permit Issued" only after you pay and return the signed copies.
- **Fake documents can end your career.** DepEd warns that falsified qualifications are Serious Dishonesty under Civil Service Commission (CSC) Resolution No. 2100079, Section 3.

## Who could apply for NASH Batch 2

DM-OUHROD-2025-3318 named two groups of takers. The first group could take Batch 2 without meeting the qualification standards (QS) for School Principal I:

- Head Teachers
- Assistant School Principals
- FY 2021 NQESH takers under Category C
- FY 2023 NQESH takers under Categories B and C

Item 6 of [DepEd Memorandum No. 097, s. 2025](https://www.deped.gov.ph/2025/10/21/october-21-2025-dm-097-s-2025-results-of-the-national-assessment-for-school-heads-for-fiscal-year-2025-batch-1/) also let Head Teachers and Assistant School Principals rated "With Developmental Areas" in Batch 1 take Batch 2.

All other personnel had to meet the new CSC-approved QS for School Principal I under [DepEd Order No. 019, s. 2025](https://www.deped.gov.ph/2025/06/24/june-24-2025-do-019-s-2025-amended-qualification-standards-for-teacher-i-iii-master-teacher-i-iv-and-school-principal-i-iv-positions-and-the-qualification-standards-for-newly-created-t/). In short, that means a relevant master's degree and five years of teaching plus one year of relevant experience. It also means 32 hours of relevant training within the last five years and RA 1080 (Teacher) eligibility.

Three more rules from the memorandum and the FAQs affect who applies where:

1. Central Office and Regional Office personnel had to meet the QS and coordinate with the SDO where they were previously assigned or served.
2. SDO evaluators could not take the NASH, to avoid a conflict of interest, unless they formally waived their evaluator role before applying.
3. FY 2023 Category C takers without a master's degree could upload the transcript or diploma of their highest educational attainment, or proof that they are currently taking a master's program.

The full eligibility rules and the QS table are in the [NASH guide](https://nash.com.ph/nash). For detailed questions on the QS, such as which master's degrees count or how coordinatorships are credited, read the [NASH qualification standards FAQ](https://nash.com.ph/blog/nash-qualification-standards-faq).

## What you need before you start the NASH application

The NASH-OAS Guide for Applicants lists what you need before you open the system. Check these items weeks before an application window opens, not on the first day.

### An active DepEd email account

You must register with your own official DepEd email, and it must be active and in current use. Annex 1 says the same email is used for the application and the assessment portals. If you do not have one, or you cannot sign in, ask your SDO Information Technology Officer (ITO) to create or activate it. That is the answer of FAQ 20.

Sign in to your DepEd email now and confirm that you can receive messages. Every status update in the NASH-OAS also arrives by email.

### A computer that meets the stated specifications

The guide sets these minimum device specifications:

| Item | Minimum |
|---|---|
| Operating system | Windows 10 (version 20H2 or later) or Windows 11 (Home or Enterprise); macOS 10.15 (Catalina) or newer |
| Processor | Dual-core Intel Core i3 or AMD Ryzen 3 |
| Memory | 4 GB RAM |
| Storage | At least 500 MB free disk space |
| Display | 1024 x 768 |
| Internet | 5 Mbps upload and download |

You also need a computer with an internet browser already installed and a stable connection. The guide names Chrome, Edge and Safari as examples. The guide lists a computer, not a phone, as the device for the application. A phone scanning app is still allowed for preparing documents, as explained below.

The assessment itself had its own laptop requirements, because it ran on the LMS through the Safe Exam Browser. The [NASH online assessment guide](https://nash.com.ph/nash-online-assessment) explains them.

## How to create your NASH-OAS account

Account creation and the application are two separate steps. In Batch 2, account creation also closed earlier than document submission. FAQ 22 told applicants to create the account first and upload documents later, before the submission deadline.

1. Open your browser and go to https://nash.deped.gov.ph.
2. Sign in with your own DepEd Microsoft or Google account. The guide says Google is preferred.
3. A message says you do not have an account yet. Click "OK".
4. Read the Privacy Notice. If you agree, click the underlined sentence to open the Create Account page.
5. Fill in the applicant information and work information (see the table below).
6. Click "Create". A window asks you to confirm your details. Check every field, then click "OK".
7. A "Successfully Registered" message appears, and a confirmation goes to your DepEd email.

| Field | What to enter |
|---|---|
| Name | First, middle and last name, and name extension. Leave the middle name or extension blank if you have none |
| Date of birth and sex | Select from the calendar and the list |
| Mobile number | Format 09XXXXXXXXX; landline with area code, if any |
| Number of times taken | 0 if this is your first time; otherwise the number, plus the years taken from the list (Principals' Test 2016 to 2018, NQESH 2021 and 2023, the 2021 Category B evaluation, NASH Batch 1) |
| Plantilla position | Select from the list, or choose "Other Position" and type it |
| Designation | Select from the list |
| Governance level | Central Office, Regional Office, Division Office or School, then the region, division and office or school name |

### Why your account details cannot be changed

Annex 2 states in capital letters: "ANY INCORRECT DETAILS CANNOT BE CHANGED". The reason is that your account feeds three later steps. The guide says the information "will be used for enrollment in the DepEd Learning Management System (LMS), as well as for taking the assessments and viewing your results."

A misspelled name can therefore follow you to your LMS account and your Individual Certificate of Rating. Type your name exactly as it appears on your service record and PRC license. Check the region and division too, because they decide which office receives your application.

If you still make a mistake, FAQ 21 gives the only route. Coordinate with your Regional HRMO, through your assigned SDO evaluator, to request the change. You must do it before the SDO submits the final list of takers to the Regional Human Resource Development Division (HRDD).

## NASH documents to submit: the requirements list

Annex 1 of DM-OUHROD-2025-3318 lists the documents. Each must be a clear scanned copy saved in PDF format. The NASH-OAS shows the first six as required and the rest as "only if applicable".

| Document | Who signs or certifies it | Notes |
|---|---|---|
| Approved RPMS IPCRF with at least Very Satisfactory in the last two consecutive rating periods | Authorized personnel in the SDO | For Batch 2: SY 2023-24 and SY 2024-25 (school-based) or FY 2023 and FY 2024 (non-school-based). Upload only Part I, with the rating (FAQ 26) |
| Service Record | Administrative Officer V of the SDO | Shows your teaching years and positions |
| Certification of relevant experience | Schools Division Superintendent (SDS) or School Head | Must name the area: learning area coordination, subject area supervision, school management and operations, or instructional supervision |
| Transcript of Records or Diploma | Your school | For MAEd graduates whose CHED Special Order is still pending (no S.O. number on the TOR), a certification of completion from the school |
| Certificates of participation in training | LAC and INSET certificates: the SDS or School Head | Curriculum, Pedagogy, School Management and Operations, or Instructional Leadership, within the last five years |
| PRC License or Certificate of Eligibility or Rating | The issuing office | Must be valid and updated |
| Certificate of Employment, if you taught in a private school | Head of the private school, with the official seal | Must state position, subject taught, inclusive dates and full-time or part-time status |
| OPCRF, if you are a TIC, OIC or Head Teacher acting as school head | Annex 1 names the form only | The Office Performance Commitment and Rating Form |
| Designation or Special Order as School Head, TIC or OIC | Schools Division Superintendent | Must be for a public school |

The Individual Performance Commitment and Rating Form (IPCRF) is the rating form of the Results-Based Performance Management System (RPMS). TIC means Teacher-in-Charge and OIC means Officer-in-Charge.

### Notes on the harder documents

**Relevant experience.** FAQ 18 says the certification must "clearly indicate" your involvement in one of the four areas. A general certificate of good standing is not enough. FAQ 17 adds that coordinatorships held before [DepEd Order No. 005, s. 2024](https://www.deped.gov.ph/2024/04/29/april-29-2024-do-005-s-2024-rationalization-of-teachers-workload-in-public-schools-and-payment-of-teaching-overload/) count toward learning area coordination, and shorter roles may be added together to reach one year.

**Private-school experience.** FAQ 15 and FAQ 16 confirm that private-school teaching and private-school management can count. The Certificate of Employment must carry the details in the table, the official seal, and the signature of the school head or authorized official. For management experience, it should also list your duties and responsibilities.

**Training.** Certificates from Learning Action Cell (LAC) sessions and In-Service Training (INSET) are valid when signed by the SDS or School Head. FAQ 19 says the five-year period is counted from the date of your last promotion.

## Scanning rules for NASH-OAS uploads

Annex 1 and the guide give the same scanning rules:

1. Use a scanner or a mobile scanning app. The documents name Adobe Scan, Microsoft Lens and CamScanner as examples.
2. Scan at a minimum resolution of 300 DPI.
3. Keep each page flat, well lit, and free from shadow or glare.
4. Save each document as a separate PDF with a clear file name. The guide's example is LASTNAME_Firstname_IPCRF.pdf.
5. Check that all text is legible, complete, and not cropped or blurred.

FAQ 25 sets the limit: each file must not exceed 10 MB. If a file is larger, FAQ 26 says to resize or compress it. For the IPCRF, upload only Part I with the rating, not the supporting documents.

A practical example: a Head Teacher in a large high school has a 14-page IPCRF with attachments. She uploads only Part I, two pages at 300 DPI, and the file is well under the limit. Her training certificates, scanned one by one, go into a single PDF named for that document type.

## How to submit your application in the NASH-OAS

1. Sign in again at https://nash.deped.gov.ph with your DepEd account.
2. On the home page, open the menu on the left.
3. Click "+ Apply".
4. Fill in all required fields and upload each document in its slot.
5. Confirm that you selected the correct region and Schools Division Office.
6. Click "Submit Application".
7. A message asks you to wait at least five minutes for the confirmation email before you try to submit again. Click "OK".
8. A "Successfully submitted your application" message appears. The system then opens your dashboard.

Step 5 matters more than it looks. Annex 2 warns: "Incorrect selection may result in your application being sent to the wrong office, causing delays and leading to extra processes for proper handling." Your application goes to the SDO you choose, not the SDO where you work.

## NASH-OAS application status: what each one means

After you submit, the NASH-OAS shows your status on the dashboard. Each change also sends an email to your DepEd account. This table follows Annex 2, Part I.C.

| Status | What it means | What you do |
|---|---|---|
| For SDO Review | Your application went to the SDO you selected, for review by the assigned SDO evaluator | Watch your email and dashboard. No action unless the SDO contacts you |
| For RO Validation | The SDO endorsed your application. The RO validator checks it before payment | Wait for the result. Prepare your original documents and photos |
| For Verification and Payment | The RO validated your application. You must visit the SDO | Go to your SDO with your originals, photos and the fee |
| Assessment Permit Issued | Your permit is processed and your payment is recorded in the NASH-OAS | Check that the payment details are stamped. Keep the permit for the mock and the actual assessment |
| Re-Apply | Some documents are incomplete, invalid or do not meet requirements, or you chose the wrong region or division | Read the remarks, fix the problem and submit again through the NASH-OAS |
| Disapproved | Your application did not meet the conditions set by DepEd | Read the reason in the remarks and prepare for the next NASH application |

"Re-Apply" is a correction, not a rejection. "Disapproved" means you did not meet the conditions for that batch, for example the QS. Its remarks tell you what to fix before the next administration.

## Claiming the NASH assessment permit at the SDO

When your status shows "For Verification and Payment", you complete the process in person. Bring:

- the original copies of every document you uploaded;
- two passport-size photos with name tag, taken within the last six months;
- the assessment fee of PHP 720.00.

Annex 2 and the guide describe these steps:

1. Give your photos and original documents to the SDO evaluator. The evaluator compares them with your uploads.
2. The evaluator prints your assessment permit and returns your originals.
3. The evaluator signs the permit and attaches your ID photos.
4. You sign the permit in the space for the applicant.
5. You pay the fee at the SDO cashier.
6. The cashier records the payment details and signs the permit. The signed permit serves as your Acknowledgment Receipt.
7. You return the upper portion of the permit to the SDO evaluator. These are the copies for the SDO and the RO.
8. The evaluator tags you as "Paid" in the NASH-OAS.
9. Your status changes to "Assessment Permit Issued". This confirms that you are on the Batch 2 list.

The printed form has three parts: the Registration Record (copy for the SDO), the Validation of Submitted Documents (copy for the RO) and the Assessment Permit (your copy). Keep your copy safe. At the Batch 2 sites, the facilitator's kit held the rooming list and the RO copy of your registration. DepEd added in May 2026 that a taker without a permit could be identified by ID and checked against the official room list, but do not rely on that.

## Where the PHP 720 NASH assessment fee goes

Annex 5 of DM-OUHROD-2025-3318 explains the use of the fee:

| Part of the fee | Amount | Use |
|---|---|---|
| Legal Research Fund | PHP 10.00 | Collected under [DepEd Order No. 14, s. 2015](https://www.deped.gov.ph/2015/04/23/do-14-s-2015-authorizing-the-collection-of-fees-for-the-legal-research-fund-lrf/) and remitted to the Bureau of the Treasury |
| Administration of the NASH | PHP 710.00 | Preparation, administration and post-administration costs |

Annex 5 lists these uses of the PHP 710, "but not limited to":

- board and lodging, meals, communication, travel, honoraria and uniform shirts of the regional and division working groups, monitors and site personnel;
- lunch for NASH takers on the mock and the actual assessment days;
- supplies and materials, and printing of NASH materials and forms;
- assessment site rental and electricity;
- semi-expendable items below PHP 50,000.00, and other incidental expenses.

The Regional Director approves the use of the funds under budgeting, accounting, auditing and procurement rules. Unused collections go to the Bureau of the Treasury after eligible expenses are settled. The Batch 2 documents do not mention refunds, so ask your SDO before you pay if you are unsure you can attend.

## The evaluation and validation chain

Your application passes through four hands before your name reaches a room list.

| Step | Who | What they check or do |
|---|---|---|
| 1. SDO evaluation | SDO evaluators: the SDO HRMO and others authorized by the Schools Division Technical Working Group (SDTWG) Chair | The criteria in the cover memorandum, the authenticity and completeness of uploads, and compliance with Annex 1. They forward the application to the RO, or mark it "Re-Apply" or "Disapproved", with remarks |
| 2. RO validation | RO validators: the RO HRMO and others authorized by the Regional TWG (RTWG) Chair | Review the application and the SDO's assessment. They approve it ("For Verification and Payment"), or mark it "Re-Apply" or "Disapproved" |
| 3. Verification and payment | SDO evaluator and SDO cashier | Compare originals with uploads, print the permit, collect the fee and tag you as "Paid" |
| 4. Final list and sites | SDO, RO and the Regional HRDD | Submit and approve the final list, then issue the Regional Memorandum on sites and rooms |

The validator guides show what the evaluators see. They open your application number, read your details "thoroughly until the bottom of the page", and open each attachment. A blurred page, a missing signature or a file in the wrong slot is easy to spot there.

### The final list of takers and the Regional Memorandum

After payment, Annex 2, Part IV sets the endorsement process:

1. The SDO evaluator sends the RO validator the RO copies of the permits and the final list of takers, signed and approved by the SDS.
2. The SDO cashier turns over the collected fees to the RO cashier with a master list of payments. The RO cashier issues the Official Receipt.
3. The RO validator checks the submission and forwards the final list to the Regional HRDD.
4. The RTWG Chair, through the Regional HRDD, releases the final list of takers with their assigned sites and rooms through a Regional Memorandum.
5. The Regional HRDD reports the final list, with assessment numbers, to the Central Office.

A February 11, 2026 advisory added detail. The SDS signs a printed final list. The SDO then submits the CSV file, the signed hard copy and the RO copies of the validated documents forms to the RO, through the Regional HRMO.

For you, the practical point is this: watch your Regional Office's memoranda for the list of takers and your site. DepEd counted 24,069 declared Batch 2 takers at 65 assessment sites. How the assessment day itself ran is covered in the [NASH online assessment guide](https://nash.com.ph/nash-online-assessment).

## Common NASH-OAS problems and how to fix them

The official FAQs (questions 20 to 29) and the December 19, 2025 advisory give these fixes.

| Problem | What to do |
|---|---|
| No DepEd email, or you cannot sign in | Ask your SDO Information Technology Officer to create or activate it (FAQ 20) |
| Wrong personal details in your account | Ask your Regional HRMO, through your SDO evaluator, before the final list goes to the Regional HRDD (FAQ 21) |
| Wrong region or division selected | Contact your Regional HRMO through your SDO evaluator. Your application is tagged "Re-Apply" so you can submit to the correct office (FAQ 23) |
| Wrong document uploaded | Contact your SDO evaluator so the application is tagged "Re-Apply", then submit again (FAQ 24) |
| File larger than 10 MB | Resize or compress it. For the IPCRF, upload Part I only (FAQ 25 and 26) |
| Cannot attach or upload a file | Send a screenshot of the error to support.usdlms@deped.gov.ph or use the support form linked in the guide (FAQ 27) |
| No email notifications | Report it, with a screenshot, to the same support address or form (FAQ 28). Check your spam folder first |
| Error message on submission | Clear your browser's cached data for the last 24 hours, or use an incognito window (FAQ 29) |
| You submitted but saw no confirmation | Wait at least five minutes for the email before you submit again (Applicant Guide) |

Batch 2 also showed that the system can be slow at the start. The December 19, 2025 advisory extended the application, evaluation and payment schedule. It gave the reason as "technical difficulties experienced by applicants during the initial two (2) weeks of the application period." Do not wait for the last day. Technical concerns on the NASH-OAS go to the DepEd Information and Communications Technology Service (ICTS) team at support.usdlms@deped.gov.ph. General NASH questions go to the BHROD-HRDD at bhrod.hrdd@deped.gov.ph.

## Warning on falsified NASH documents

Annex 1 and the Applicant Guide carry the same warning. "Falsifying qualifications or submitting fake documents is a serious offense under Civil Service Commission (CSC) Resolution No. 2100079, Section 3, classified as Serious Dishonesty."

Annex 1 lists what the violation can lead to: dismissal from service, cancellation of civil service eligibility, perpetual disqualification from public office, forfeiture of retirement benefits, and a ban on taking civil service examinations.

The SDO evaluator compares every upload with your original documents, so a changed rating or an invented training will be found. A certification signed by someone without authority, or a certificate for training you did not attend, carries the same risk. Upload only documents that you can show in the original.

## Checklist: prepare for the next NASH application now

You can prepare most of the requirements before DepEd announces the next administration.

- Sign in to your DepEd email and confirm that it works. Fix any access problem with your SDO ITO.
- Check that your name on the service record, PRC license and transcript is spelled the same way.
- Request a certified service record from the SDO Administrative Officer V.
- Get your IPCRFs for the last two rating periods, with ratings of at least Very Satisfactory, certified by the SDO. Scan Part I only.
- Ask the SDS or your School Head for a certification of relevant experience that names the area and the dates.
- Get a Certificate of Employment, with seal, for any private-school service.
- Collect training certificates from the last five years in Curriculum, Pedagogy, School Management and Operations, or Instructional Leadership, and add up the hours.
- Secure your transcript or diploma, or the school's certification if your CHED Special Order is pending.
- Check that your PRC license is valid and updated.
- If you are a TIC, OIC or Head Teacher acting as school head, get your OPCRF and your designation or Special Order.
- Scan each document at 300 DPI into its own PDF under 10 MB, with a clear file name.
- Set aside PHP 720 (the Batch 2 fee) and two passport photos taken within six months.
- Check that your laptop meets the minimum specifications.

While you wait, study the content. The [NASH study guide](https://nash.com.ph/blog/nash-study-guide-ppssh) explains the Philippine Professional Standards for School Heads (PPSSH) that the NASH covers. The [NASH practice questions with answers](https://nash.com.ph/blog/nash-practice-questions-with-answers) show the situational item style. Past results are on the [NASH results page](https://nash.com.ph/nash-results), including [FY 2025 Batch 2](https://nash.com.ph/nash-results/fy-2025-batch-2).

## Frequently asked questions

### What are the requirements for the NASH application?

The FY 2025 Batch 2 requirements were an active DepEd email and PDF scans of your IPCRF, service record, certification of relevant experience, transcript or diploma, training certificates and PRC license. TICs, OICs and Head Teachers acting as school heads also uploaded their OPCRF and designation. Applicants with private-school service added a Certificate of Employment.

### What is the NASH-OAS website?

The NASH-OAS is at https://nash.deped.gov.ph. It is DepEd's National Assessment for School Heads Online Application System. You sign in with your official DepEd Google or Microsoft account to register, upload documents and track your status.

### How much is the NASH assessment fee?

The FY 2025 Batch 2 assessment fee was PHP 720.00, paid at the SDO cashier after your application was validated. Of this, PHP 10 went to the Legal Research Fund and PHP 710 to administration costs, including takers' lunch on the mock and actual assessment days.

### Can I edit my NASH-OAS account after I create it?

No, you cannot edit your account details in the system. FAQ 21 says to request corrections from your Regional HRMO, through your SDO evaluator, before the final list is submitted to the Regional HRDD.

### What does "Re-Apply" mean in the NASH-OAS?

"Re-Apply" means the SDO or RO found a document incomplete or invalid, or you selected the wrong region or division. Read the remarks, fix the problem and submit your application again through the NASH-OAS.

### When is the next NASH application?

DepEd announces each NASH administration through a memorandum. The FY 2025 batches ended with Batch 2 in May 2026, after two reschedules. Watch DepEd and your Regional Office for the next memorandum, and prepare your documents now with the checklist above.
